Adult Measures for adults 18 years and older

The OQ®45.2 is a 45-item self report Outcome Questionnaire.

The OQ®-30.2 is a 30-item Outcome Questionnaire which is briefer than the OQ®-45.2.

OQ®-WRAP (Wellness and Resilience Assessment Post-deployment) is a selection of questions from the OQ®-10 and the OQ®-ASC instruments developed by the Madigan Army Hospital with special permission from OQ Measures, LLC to treat our troops post-deployment.

 

Adult Measures for the severely mentally ill population

S®-OQ 2.0 is a 45-item Outcome Questionnaire designed for the SMI (severely mentally ill) population.

BPRS an 18 item Measure to be used in conjunction with the S-OQ® 2.0.

Youth Measures for children and adolescents

Y-OQ®2.01 is a 64-item Outcome Questionnaire administered to parents/guardians of adolescents ages 4 through 17.

Y-OQ®SR 2.0 is a 64-item self report administered to adolescents ages 12 through 17.

Y-OQ®30.2 (Omni form) is a 30-item Outcome Questionnaire administered to the client or parent/guardian for ages 4 through 17.

The Y-OQ® ASC Youth and Parent instrument is used in conjunction with the OQ®-45.2.
