OQ Measures LLC distributes the following paper and pencil questionnaire products each of which were designed and developed to be reliable and valid.

OQ® Adult Measures:

OQ®-45.2 A 45-item self-report, three subscale outcome and progress tracking measure designed to be repeatedly administered to assess ongoing patient change.

OQ®-30.2 A 30-item, single-subscale, self-report outcome and progress tracking measure designed to be repeatedly administered to assess ongoing patient change.

OQ®-10.2 is a ten item derivative of the OQ®-45. It is used as a quick screening measure and a tool to track the mental wellness of populations.

Y-OQ® Youth Measures:

Y-OQ® 2.01 Is a parent report outcome and tracking measure designed to be repeatedly administered to assess ongoing patient progress.

Y-OQ® SR 2.0 is a youth self report outcome and tracking measure designed to be repeatedly administered to adolescents to assess their ongoing progress in treatment.

Y-OQ® 30.1 (omni) A 30- item, single-subscale, self- report or parent \ guardian report outcome and progress tracking measure designed to be repeatedly administered to assess ongoing patient change.

Paper & Pencil Products in Other Languages

All measures are available in other languages for a fee of $25 per language. Simply list the desired language on section B of the License Agreement order form and it will be included in your shipment. If the measure has not already been translated into the desired language additional fees may apply. Please see the list below for current availability or contact OQ Measures for assistance.

OQ-45 is available in English, Spanish, French, Japanese, Swedish, Dutch, French, Norwegian, Arabic, German, Italian, Russian.
OQ-30 is available in English, Spanish, French.
OQ-10 is available in English, Spanish, French.
YOQ2.01 is available in English, Spanish, French, Swedish, Dutch.
