A product of over two decades of research by Dr. Michael Lambert, the OQ®-45.2 is a self-report Outcome Measure designed for repeated measurement of client progress while in therapy and following termination.

The OQ®-45.2 measures three subscales:

1 Symptom Distress (depression and anxiety)

2 Interpersonal Relationships (loneliness, conflict with others and marriage and family difficulties)

3 Social Role (difficulties in the workplace, school or home duties)

You can use the subscale scores to identify and target particularly problematic areas as a focus of treatment.

Risk assessment: The OQ®-45.2 contains risk assessment items for suicide potential, substance abuse, and potential violence at work.

Standardized data: The OQ®-45.2 is a standardized instrument with empirical support. It is based on normative data.

Content that assesses functional level and change over time: The OQ®-45.2 measures functioning in 3 domains: Symptom distress (heavily loaded for depression and anxiety), Interpersonal functioning, and Social Role. It enables the clinician to assess functional level and change over time.


The OQ®-45.2 is available in English, Canadian French, Norwegian, Spanish and Swedish with the OQ®-Analyst software.

The OQ®-45.2 may be administered and scored in a matter of minutes.

The information provided by the OQ®-45.2 Outcome Questionnaire is most meaningful when it is first administered to a patient prior to applying any therapeutic interventions.

Remember that any intervention, even an intake interview, is likely to cause client improvement; therefore, delaying administration of the OQ®-45.2 will result in an underestimation of treatment effects.

Since the OQ®-45.2 was designed to take only 3-5 minutes to complete, taking the test on multiple occasions does not place any additional burden on your client. Weekly administrations are recommended but subsequent administrations may be given at any determined midpoint intervals, and at the conclusion of treatment.

Since routine treatment ends with your client leaving treatment, data collected on an interval less than weekly will result in loss of data. While information about improvement following a specific session may be very meaningful, perhaps more important to you is your ability to see the patterns and trends exhibited by your clients during therapy. This is accomplished by weekly administration and a review of the current client report prior to the session.

The OQ®-45.2 TA is a new measure which includes the OQ®-45.2 and 11 questions from the OQ®-ASC about the Therapeutic Alliance.

The goal of the 11 Therapeutic Alliance questions is to assess your client's perception of the relationship between you and your client.

While it may be reassuring to know that clients have very positive feelings about their relationship with you, it is more helpful in a problem-solving mode to identify ruptures in the relationship and move to repair them.

The OQ®-45.2 TA is available in English and Spanish with the OQ®-Analyst software.


A basic license entitles you to three instruments and will cover up to 200 clients/patients per year.
